Your privacy. Your data. Your rights. Whether you’re a patient or a staff member, a volunteer or a fundraiser, we respect your right to privacy.

  • There are many reasons that we may hold information about you. Please be assured that any personal data we have is safe and secure and we will never sell personal information about you to a third party.

We value your support and respect your privacy.  Hospiscare is fully compliant with the UK Data Protection Act 2018 and General Data Protection Regulation 2016.

Patient Confidentiality

All confidential information is treated respectfully. However, some information may be shared in order to provide care and treatment. We will not disclose any information without your consent unless there are circumstances where current legislation permits us to do so. Data protection – your individual rights

Right to be informed

You have the right to be informed about the collection and use of your personal data.  The above privacy notices and this web page together outline how we use and collect your personal data when you become involved with our services and activities.  Our cookie policy outlines how we collect your data when you use this website.

Right of access – Subject Access Request

You have the right to see any of your personal data being processed by us.

You are also entitled to be informed why we are processing your information, where it came from and who might be shared with. However, there are some exemptions where this right of access does not apply.

If you would like to obtain a copy of part or all of your health record, please complete the request for access to: health records form.

Staff members (including volunteers) and Supporters who wish obtain a copy of their record may apply using the request to access: personnel/supporters records form.

Requests do not have to be made using these forms, however, we will not process any requests unless:

  • The request has been put in writing
  • We are satisfied as to the identity of the person making the request
  • You provide us with enough clarity and detail to find the information that you are requesting

You are entitled to receive a response within one calendar month.

Right to correct or remove incorrect data (rectification)

If you believe your personal data is incorrect or inaccurate, please contact us identifying the information you feel is incorrect and what should be done to correct it.

Right to be forgotten (erasure)

You have the right to request that your personal data is erased from our systems. This is rights only applies in certain circumstances for example where we no longer need to process your information for a specific purpose.

Right to restrict processing

You have the right to prevent your personal data being used for direct marketing purposes. You will need to write to us as the data controller requesting that we stop processing your personal data for direct marketing purposes. Right to prevent automated decision making

You can request that the we limit the way personal data about you is used, where you have a particular reason for wanting the restriction, for example, if you feel information about you is incorrect.

Right to data portability

You have the right to obtain and reuse your personal data for your own purposes across different services and organisations. However, this right only applies where personal data has been provided by you and the processing is based on your consent.

Right to object

You can object to us processing your personal data, where you have an objection based on ‘grounds relating to your particular situation’.

Automated decision making, including profiling

You have the right not to be subject to a decision based solely on automated means, including profiling where the process produces a legal effect or has a significant impact.
