Volunteer Care Navigator

This is an unpaid voluntary role

Can you help your local hospice raise vital funds for our specialist care?  

Location: Some patients are supported over the telephone, some in their own home and some in the community.

We are currently looking for Care Navigators in Axminster, Seaton, Honiton, Ottery, Tiverton, Cullompton and Okehampton.

Typical activities include: 

  • Support individuals over the telephone.
  • Support individuals or families at home or in the community.
  • Support our patients who have dementia.
  • Support our patients and families, particularly when the patient has something specific they need more support with. For example, this could be:
  • Talking about the plans for the end of their life.
  • Helping them thinking about their funeral.
  • Helping them to realise final hopes, e.g. to see the sea again.
  • Helping with admin/IT tasks.
  • Signposting to transport and other local services.

 

What we’re looking for Someone who can:  

  • Be sensitive, flexible, empathetic and non-judgemental
  • Cope well with those who are in distressing and emotive situations
  • Use Zoom to attend supervision and team meetings
  • Use and develop good listening skills
  • Listen without counselling or offering advice
  • Be reliable
  • Attend monthly meetings
